The following documents provide the application and supporting materials for the Chronic Care Improvement Program.

Phase I
Proposals for Phase I of the Chronic Care Improvement Program are due August 6, 2004. At minimum, the proposals should include a cover letter, the application, an executive summary, and a problem statement explaining the rationale for the proposed geographic area and target population.

Chronic Care Improvement Application (PDF file)
CMS requires applicants to use the waiver demonstration application for the Chronic Care Improvement Program even though it is not a demonstration program. Applicants must follow the format of the the waiver application and include the information requested in the Federal Register notice. See 69 Fed. Reg. 22065, 22072-79 (April 23, 2004).

Data Use Agreement Application (PDF file)
This is the data use agreement for a sample data set, which CMS encourages applicants to obtain prior to submitting their proposals. The original appeared at the CMS Web site, but is no longer available. The Web site stated that questions regarding the data set should be addressed to the Research Data Assistance Center at https://resdac.oit.umn.edu/.
